It has been a tremendous privilege to serve as your physician and to participate in your healthcare over the years. The trust you have placed in me and the relationships we have built have been among the most meaningful aspects of my professional life.
After careful consideration, I have made the difficult decision to suspend all surgical procedures and take an extended leave of absence starting October 31, 2026.
To ensure continuity of your medical care, I encourage you to establish care with another physician as soon as possible.
Patients needing assistance in locating another physician should contact their health plan to find an in-network provider. Other sources to use are:

Your Medical Records
Your medical records will be securely maintained for at least 10 years from your last date of service, in compliance with applicable laws, and are available at Morgan Records Management, LLC, the official records custodian.
To request a copy of your records
Submit a records release form with Morgan Records Management at www.MorganRecordsManagement.com, then:
- Select Request My Records
- Select your state & practice
- Complete the required fields
